Two Incredible Equation Forms

We are making our very functional incredible equation form available. It's a 'tried and true' form that we use in our classroom. There are two forms - one for first grade that features four concept areas (word problem, number sentence, groups/place value, and money) and one for second grade  with six concept areas (all of the first grade areas plus missing parts, and time in digital and analog). Students write the number of the day in the star in the center.

Two Daily Math Log Forms

This is a great document for daily review of four math strands - money, time, problem solving, and place value. We've created two Daily Math Log forms -- one for K-1 and one for late 1st or 2nd grade. We've included teacher notes describing how we use this in our classroom. Laminate this colorful form and use with dry erase markers for a durable tool that will last for years.

2, 3 and 4 Column Graph Pack with Reflection Page

This graph set includes a form for 2, 3, and 4-column graphs. Included with each graph template is a reflection page that asks students to write, draw and use numbers to tell about the graph. You'll find this kit to be very useful in your classroom. Create transparencies and use them to model "think-alouds" as you introduce or review graphing and problem solving strategies.

Use this reading log to encourage reading at home. Spaces are provided for the date, title, minutes read, and parents' initials.

Anecdotal Notes Form

We've shared two of our anecdotal recording keeping forms. One with language arts focus areas and one for math. These forms are designed for use with 1 1/2 inch by 2 inch sticky notes. Keep a pad in your pocket and jot notes as you work with children. Later, put the notes on students' form. Photocopy the page (or slide it in a sheet protector) to avoid losing the sticky notes.
